Financial Policies

As patients approach surgery, they frequently need information about the various payment options and have questions about their potential insurance benefits.  Please use the following information to make your payment arrangement.

We are readily available to provide the specific information you desire.  We will use our expertise to help you obtain the maximum benefits from your insurance policy.  Please be aware, routine follow-up care for the first ninety (90) days is covered within your initial surgery fee.  Any services provided beyond ninety (90) days may be subject to a charge.

Payment for cosmetic surgery is due, in full, two (2) weeks prior to surgery.

We accept the following methods of payment:

  • Personal check, Cashier's check, or Cash
  • Visa, MasterCard, Discover, or American Express
  • Optional financing can be recommended, if requested.

If surgery arrangements are made with less than two (2) weeks notice, all surgery fees must be paid within 48 hours of the surgery reservation by any method listed above, excluding personal checks.

*Without receipt of timely payment, your surgery date and time may be forfeited.*

Patient Cancellation of Cosmetic Procedures

We understand you may have a situation which forces you to postpone your surgery.  Please understand such changes affect not only your surgery, but the surgery facility and other patients as well.

If you cancel within ten (10) or more business days before surgery, a full refund will be given less any credit card processing fees.

Should you find it necessary to cancel surgery nine (9) to four (4) business days prior to surgery; a refund of 75% of the total fees will be paid to you.  If you cancel within three (3) to two (2) business days, a refund of only 50% of the total fees will be paid to you.  If you cancel within one (1) business day, a refund of only 25% of the total fees will be paid to you. Should be decide to reschedule surgery, any forfeited monies may be applied to your rescheduled surgery.

Scheduling

If a pre-determination letter is submitted to your insurance company for a procedure on your behalf, the surgery may be scheduled no fewer than six (6) weeks from the date of letter submission.  In the event approval for the procedure has not been received by one (1) week prior to the scheduled surgery, the surgery will be rescheduled for no fewer than four (4) weeks from the date of rescheduling.  The surgery may be scheduled sooner if the pre-determination approval letter is received, the surgeon is available, and the patient desires to move up the surgery date.  You also have the option to pre-pay for the surgery in the event you do not wish to wait on the insurance company's response.

Paperwork

Any extensive paperwork completed by our office secondary to your surgical procedure, such as FMLA, will be completed free of charge.  Any further paperwork required will incur a $15.00 charge.
